And while the Chicago Cab soundtrack probably isn't as widely known, the 10 club was including free copies of it in every order for years.

"Out of My Mind" is definitely a true b-side. A lot of fans at Philly 4 in 2009 didn't even know it haha.
I guess by the criteria established for b-sides, though, not many are really eligible. Most are not that elusive any more with the Internet, Lost Dogs, live recordings, etc. It's fair to just say anything that's not on a studio album, I think.
As for covers, here are my top 10:
