they haven't released the cause of death yet, as far as i've seen.  you have to suspect that there was some complication or other relationship between the leg/hip injury and his death but nothing confirmed yet.

they were.  i caught their 9/13/2009 and 9/14/2009 shows, but unfortunately wasn't around for 2016's Surprise! At the Club! performance.
